Supported Living Services (SLS)

KAAUM's Supported Living Services (SLS) are designed to help adults with I/DD live as independently as possible while receiving the support they need to thrive. This program offers flexible services tailored to the individual’s needs, abilities, and goals—all delivered in home and community-based settings.

Personal Care

Short-Term Respite

Personal Care

Personal Care services offer hands-on assistance with the most personal daily living tasks that the individual may not be able to perform on their own. These tasks may include:


  • Bathing, dressing, grooming
  • Toileting and mobility
  • Eating and medication reminders


This service helps individuals maintain their health and hygiene in a supportive and respectful way.

Mentorship

Short-Term Respite

Personal Care

Mentorship services are designed to promote self-advocacy and informed decision-making. Support may include verbal instruction, sharing experiences, modeling solutions, and guidance in areas such as interviewing potential providers, navigating health and safety concerns, and participating in advisory groups or public boards.


Mentorship may also include training in child and infant care for individuals with I/DD who are parents, supporting their ability to confidently care for their families.

Short-Term Respite

Short-term respite services are designed for two purposes: to provide relief to a primary caregiver, or to offer individuals living independently access to occasional, structured activities in the home or community. This service ensures safe engagement and continuity of support to provide enriching time in a familiar or social setting.


These services are only available during the day and are not provided on an overnight basis.

Homemaker (Enhanced)

Basic homemaker services provide assistance with routine household tasks to help maintain a clean and safe living environment in the residence. 


Basic services may include:


  • Light housekeeping
  • Vacuuming, mopping & dusting
  • Washing laundry and bedding
  • Garbage disposal and mail retrieval
  • Meal planning & preparation
  • Grocery shopping assistance


Please note: Homemaker services do not include personal care, tasks the member can perform independently, services provided by family members, or services delivered in uncertified congregate facilities.

Homemaker (Enhanced)

Enhanced services include all aspects of Basic Homemaker support, as well as some additional services like extraordinary cleaning due to behavioral needs or medical conditions.


In addition, enhanced homemaker services include training and habilitation, using specific goals to increase independence in household tasks. This may include step-by-step instruction, verbal prompting, and partial physical assistance.


Examples of additional/extraordinary services offered under enhanced homemaker services:


  • Deep cleaning in kitchen and bathroom
  • Support in monitoring diet and nutrition
  • Assistance with sorting and reading mail
  • Proper disposal of garbage and soiled items
  • Home maintenance supervision

Is SLS Right for YOU?

Who are Supported Living Services for?

SLS is intended for individuals who do not require 24/7 care but need help with certain tasks to maintain independence. To qualify for these services, individuals must be enrolled in the SLS Medicaid waiver. These supports allow individuals to live in their own homes, with family, or with roommates, while receiving assistance that promotes health, safety, and community inclusion.

Flexible Support

SLS services are highly individualized and based on the person’s preferences, strengths, and support needs. The frequency and type of support vary, allowing each person to build a schedule that works for their lifestyle and goals.

Family Involvement

Family members often play an important role in the lives of individuals receiving SLS. In some cases, eligible family members can become approved caregivers and be trained to provide direct support. We value family involvement and work to create collaborative care plans that support both the individual’s goals and the family’s capacity to assist.
